Request to be removed as designated premises supervisor
If you are the designated premises supervisor on a premises licence, you can notify us that you wish to be removed. You must also give notice of this request to the premises licence holder and either return the premises licence to us or ask the premises licence holder to return it.
You should return your old licence to: South Ribble Borough Council Licensing Team, Civic Centre, West Paddock, Leyland, Lancashire, PR25 1DH
To request to be removed as the DPS you should complete the request to be removed as DPS application form.
The completed form should be sent to us by email to licensing@southribble.gov.uk. Alternatively you can post a copy to South Ribble Borough Council Licensing Team, Civic Centre, West Paddock, Leyland, Lancashire, PR25 1DH.
