Change of name or address- premises licence holder or designated premises supervisor
The premises licence holder must, as soon as reasonably practicable, notify us if they or the DPS has changed their name or address.
This notification is only in the circumstances where it is the same person or company but the name of that person or company changes (for example where the name has changed by marriage or deed poll).

You should return your old licence to us and you will need to pay the fee of £10.50. You will need to call us on 01772 625625 to make payment.
You should return your old licence to: South Ribble Borough Council Licensing Team, Civic Centre, West Paddock, Leyland, Lancashire, PR25 1DH
You will receive a new licence with the updated information.
