We have a Social Prescribing Team who are there to support you and build confidence to manage your own health and wellbeing and address any other issues that may be impacting your day to day life.
You can speak with a member of our team who will be able to understand if our service could help you, or whether we may be able to put you in touch with other services who are better placed to help.
The support we provide is free, confidential and personalised support that starts with you and guided by you. Once you have contacted us, someone will be in touch with you for a chat to get to know what we can help you with, from there we will sort out an appointment and support going forward. We want to support you in a way that works for you, so you will have the choice over if you wish to have your appointments over the phone or in person.
Here is an example of some of the issues we can support with:
- money worries
- housing issues
- isolation / loneliness
- employment / new skills
- diet / exercise
- mental health and wellbeing.

Please send us your name, a contact number and email and a brief summary of what support you need.
If the support, you need is urgent or an emergency you should contact the most appropriate service. In any event of an immediate risk to life call 999.
